It will be completed by a DDS physician or psychologist who will review the records and indicate in the form your ability to function (sit, stand, walk, crouch, stoop, etc) in quantifiable terms. When you appeal your case before an administrative law judge, you will be able to submit a RFC form completed by your personal physician or psychologist, whose opinion is given greater weight because they have such close personal knowledge of your condition.
